Consent & Liability Waiver Form
At BookSmart Tutoring, we believe learning doesn’t stop at the classroom door — it’s everywhere! That’s why we love taking our students on field trips that spark curiosity, build connections, and make memories. Our upcoming adventure to the Royal Ontario Museum is all about exploring history, science, and art in a way that brings lessons to life.
To make sure the day runs smoothly and everyone stays safe, we ask all parents and guardians to review and complete the consent form below. This ensures we’re on the same page about transportation, supervision, and those “just in case” situations we hope never happen.
1. Acknowledgment of Risk
I, the undersigned parent or guardian, acknowledge that participation in this field trip and associated transportation carries inherent risks. I agree that BookSmart Tutoring shall not be liable for any injury, loss, or damage arising from participation in this event, except where caused by gross negligence or willful misconduct. I understand my child attends at their own risk.
2. Transportation & Supervision Agreement
Once my child is dropped off at Vanessa’s house, I agree that supervision by BookSmart Tutoring begins and continues until pickup or the end of extended care. I accept responsibility for timely pickup unless Extended Care is arranged.
3. Media Release Consent
I grant permission for my child’s photograph to be used for promotional, educational, or public relations purposes, including on social media, marketing materials, or the company’s website.
4. Medical and Emergency Authorization
In the event of injury or illness, I authorize BookSmart Tutoring to arrange medical care or hospitalization for my child. I agree to be responsible for any and all costs incurred for such care.
